Sindh Institute of Ophthalmology and Visual Sciences (SIOVS) is implementing a project Cataract Support By CBM International.” SIOVS intends to receive quotations for IOLs and Consumable items details are given below, for this you are requested to submit your quotation, stating rates on below mentioned required terms. Quote should reach to the office of Deputy Director, Ophthalmic Community Survives SIOVS, Office within 07 working days.

Supplier Eligibility Requirements

Delivery Terms and Conditions

1. Delivery Location: SIOVS Hyderabad.

2. Delivery must be completed within the agreed delivery schedule after issuance of Purchase Order.

3. Transportation, loading, unloading, and safe delivery shall be the responsibility of the supplier.

4. Products must be properly packed and protected against damage during transportation.
   

Submission Instructions

1. Quotations must be submitted in sealed envelopes clearly marked as “Quotation for Intra Ocular Lenses (IOLs) and Consumable Items”.

2. Quotations should reach SIOVS on or before 25th May 2026.

3. Quotations must be submitted through courier service only.

4. SIOVS shall not be responsible for delays caused by courier services.

Financial Terms

1. Rates must be quoted in Pakistani Rupees (PKR) inclusive of all applicable taxes.

2. Payment details must be clearly stated on company letterhead.

3. Payment shall be made through cross cheque after deduction of applicable taxes.

4. Prices quoted must remain valid for at least 30 days from the closing date.

General Terms and Conditions

1. Lowest price will not be the sole basis for award of contract.

2. SIOVS reserves the right to accept or reject any or all quotations.

3. Suppliers must not engage in corrupt, fraudulent, collusive, or coercive practices.

4. Incomplete quotations or quotations received after the deadline may not be considered.

5. SIOVS reserves the right to increase or decrease the quantities mentioned in this RFQ.
