Job Summary
Background: Aga Khan Health Service, Pakistan (AKHS, P) runs one of the largest not-for-profit private healthcare systems in the country, with a focus on the regions of Gilgit-Baltistan, Khyber-Pakhtunkhwa, Punjab, and Sindh. Its core focus includes initiatives for hard-to-reach populations through services like outreach clinics, training of community midwives to provide low-cost health services in remote areas, screening for non-communicable diseases, and digital health solutions.
Aga Khan Health Service, Pakistan, is looking for a competent Registered Nurse based at Aga Khan Health Center-Gahkuch
Role and Responsibilities:
Community Outreach, Registry & Capacity Building:
- Register elderly individuals into the Elderly Care (EC) registry in line with project guidelines.
- Conduct ICOPE screenings at BHCs and during outreach activities and enroll eligible older adults into the EC roster.
- Organize and conduct community outreach camps to provide ICOPE screenings, basic counselling and lifestyle coaching.
- Ensure enrollment of registered older adults into subscribed care packages.
- Plan and deliver education and capacity-building sessions for communities and community caregivers to strengthen support for older adults.
- Support the RONAQ initiative by engaging participants and peer coaches, conducting follow-ups, and facilitating referrals when required.
Care Coordination and Referrals:
- Develop individualized care plans in consultation with clinicians based on patient needs.
- Conduct home visits and outreach activities to ensure continuity of care when required.
- Identify referral needs and facilitate timely consultations with physicians and specialized services.
- Collaborate with physicians, LHVs, caregivers, and patients to ensure appropriate medications, lifestyle modifications, and therapy services.
- Report complications, concerns, or changes in patient condition promptly to physicians.
Basic Nursing and Clinical Support:
- Perform comprehensive nursing assessments, including assessment, diagnosis, planning, intervention and evaluation.
- Conduct detailed patient and environmental assessments related to outreach and home health needs.
- Carry out assessments through ICOPE protocols of elderly patients and provide supportive nursing care.
- Administer medications through appropriate routes (oral, IV, etc.) as prescribed on a need basis.
- Identify signs and symptoms of infection and provide wound care, including long-term catheter management.
- Monitor and demonstrate effective use of electronic monitoring equipment.
Patient Education, Counselling & Psychosocial Support:
- Provide individual and family education on disease management, preventive care, and lifestyle modification.
- Offer counselling and guidance to patients and caregivers to improve adherence to care plans.
- Identify psychological or mental health concerns and facilitate referrals to specialized mental health services.
- Provide palliative care awareness and basic support to minimize unnecessary hospitalizations.
Documentation, Reporting & Compliance:
- Maintain accurate, complete, and up-to-date medical records, healthcare algorithms, and log sheets for all patient interactions.
- Adhere strictly to project protocols, clinical guidelines and standards.
- Participate in trainings provided by the project and apply learned practices in service delivery.
Qualifications & Experience:
- Diploma course for Registered Nurse. Baccalaureate Degree in Nursing (BScN), post-RN BScN will be preferred
- 1-2 years of work experience in clinical settings, trained or experienced in palliative care/geriatric nursing care, will be an asset
- Valid practicing license from the Pakistan Nursing Council (PNC)
- Certification in Basic Life Support, Safe Medication Administration, Intravenous Cannulation, NG insertion,and Foley’s catheterization
- Ability to assess, plan and manage patient care with expertise as per protocols
- Willingness to work in flexible timings
- Excellent interpersonal skills, including communication skills
- Able to use digital applications on tablets, mobile, and laptops
- Basic Computer Skills/Microsoft Office
